Leading the Way to Diabetes & Tobacco Treatment for those Diagnosed with
Behavioral Health and Substance Use Disorders

Cost: Free (a light dinner will be provided promptly at 5:00 pm)

WHO SHOULD ATTEND? Professionals that serve people with diabetes, provide DSME services, or want to increase referrals to DSME. This includes but is not limited to: certified diabetes educators, registered dietitians, registered nurses, pharmacists, physicians, advanced practice registered nurses, physician assistants, social workers, and behavioral health professionals.

  • Learn strategies to increase access to and participation in Diabetes Self-Management Education (DSME) among individuals diagnosed with behavioral health or substance use disorders.

  • Increase confidence to effectively deliver DSME programs to this population.

  • Make the association between diabetes and tobacco treatment and HEDIS performance.

  • Learn about tobacco treatment resources offered through QuitNow-NH.

Parking: Free after 3:00 pm on top level of parking garage
