Understanding the challenges and liabilities associated with documentation.
This virtual presentation "Defensible Documentation" is targeted towards all clinicians who document within the medical record, including physicians, advanced practice providers, nurses, medical assistants, social workers, techs, and others. The goal is for all clinicians to document within the medical record accurately, objectively, and comprehensively. The presentation will cover the following:
- Examine the challenges with documentation
- Discuss Liability and Standard of Care
- Review frequent allegations made against Healthcare Providers
- Recognize the liabilities associated with incomplete and inaccurate documentation
- Discuss how to document difficult interactions
- Apply strategies to improve documentation

Thom Goodwin, Senior Risk Manager, Medical Mutual Insurance Company of Maine
Thom is a Senior Risk Manager at Medical Mutual Insurance Company of Maine. He began his career in healthcare in 1983 in the clinical laboratory and began transitioning into patient safety, risk management, and medical staff governance in 2005. Thom graduated from the University of Rhode Island with a B.A. in Biology and a MS in Clinical Laboratory Science/Adult Education. He also earned a MS in Health Law/Risk Management from Nova Southeastern -Shepard Broad College of Law. He is a Certified Professional in Healthcare Risk Management (CPHRM), Certified Professional in Healthcare Quality (CPHQ), Certified in Healthcare Privacy Compliance (CHPC), and Certified in Infection Control (CIC). Thom is a member of the American Society of Healthcare Risk Management (ASHRM), Northern New England Society for Healthcare Risk Management (NNESHRM), Association for Professionals in Infection Control and Epidemiology (APIC), National Association of Healthcare Quality (NAHQ), and the Health Care Compliance Association (HCCA).
